Operative Report by Dr. Barry Eppley
Anesthesiologist says he was not present for my case.

This is the operative report for my surgery with Dr. Barry Eppley. The anesthesiologist is listed as Kevin O'Neill, M.D. Dr. O'Neill is a board certified anesthesiologist licensed in Indiana.
The problem is, he was not present for my surgery. How do I know? I have it in writing.. straight from Dr. O'Neill.
